Orientation Checklist

Necessary Items for Orientation:

You will need to make a copy of the documents, and then bring BOTH the original documents and the copies to Orientation (so we can verify their authenticity and then we’ll put the copies of the documents in your file). There will NOT be access to a photocopy machine at Orientation. NOTE: If you do not have these items at orientation, we cannot officially enroll you in the program until you provide them. 

► Documentation of citizenship (Unexpired Passport, Original or Certified Copy of Birth Certificate, or INS Certificate), plus copy

► Proof of identity (Driver’s license, military card, or DMV ID card), plus copy

►(if possible) Social Security Card, plus copy

Other Things to Bring to Orientation

► Completed Emergency Contact Information Form.

► Copy of Health Insurance Card (if you are going to be covered by other health care, as our program provides basic health care only for members not covered otherwise), plus copy

► Voided Check if you would like to do direct deposit (strongly encouraged if you will be living outside the Portland Metro area)

Members should also bring the following personal items:

► Toiletry items (towels and bedding will be provided for you)
► Casual and appropriate clothing for three days (This is Oregon! Be prepared for hot, cool, or rainy days and nights)
